For last couple of days I have been following question regarding BLOCKED Account. It felt like everyone is asking the same set of questions. I am trying to compile them together.
1. I could not find the application form?
Follow this link. Change the language to English. Click on ‘Forms’

2. I do not have a Tax ID number. What should I do ?
Firstly, you can put your National Id 19XXXXXX (NID) like this

On the other hand, you can add the abbreviation n/a (not available) in the box if you do not pay taxes in your home country.

3. My Blocked Account form has been rejected 🙁
Try to follow the email instruction. Sign every place that says account holders signature/ depositors signature.
Besides, you do not have to get German Embassy attestation this time. Just send the form to Deutsche Bank.
If you are confused whether you should send extra documents or not, my personal opinion is send your passport copy along with a print out of the email you got from Deutsche Bank.

Hello, I have submitted application for opening blocked account for international student. My documents have been delivered in Hamburg in 17th of May 2016. In 27th of May, Deutsche Bank e-mailed me that there is missing information in Tax Residence and Tax ID No. and suggested me to send the filled-up form again with signature (German Embassy attestation is not necessary). As per guideline of this e-mail I have sent the document in 29th of May which was delivered in 1 of June. I have informed them through e-mail. My Visa appointment is 20th June. I am worried that I will be able to reach the date with my bank account information.
I have tried to reach DB through hotline, they are willing to talk about this issue over phone. I e-mailed to DB online service and wrote to DB’s FB page. According the replies they also unable to answer me and it may take 25 working days.
I am confused that how long I have to wait. 25 days will be counted from 27th May or 1 June?
At this point, I think you have done what you could do. One thing you can do more is to forward your conversation/email to German Embassy Dhaka and ask for their opinion.
